Download the form that you want to use. Place your cursor on a blank field, click and begin to enter your data into the field. You can tab to the next field until the form is completed. If you are using the free Acrobat Reader, you cannot save your data so you will need to print a copy for your records and one to mail. (Hint: Most check boxes can be checked by clicking on the box or hitting the space bar.)
